Prostititis is a common misspelling of PROSTATITIS .
Chronic prostititis is divided into two main classes, chronic bacterial prostititis which is caused by chronic bacterial infection, and chronic prostititis/chronic pelvic pain syndrome, or CPPS which the cause is still not clear. Chronic prostititis symptoms are pain in the lower pelvic region of men, urinary symptoms such as frequency and urgency of passing urine. Effective treatments are difficult to find and oral antibiotics is the only treatment available, prostate symptoms may last years although they may come and go or vary in severity.

What is chronic prostititis?
Prostititis is the infection in your prostate gland, prostititis can be acute in the beginning and become chronic after a few months. Prostititis can also be caused by infection or non-infection.
How to diagnose chronic prostititis?
Symptoms must linger for three months or more, in acute prostititis, symptoms of the prostate usually come on and go away more quickly.
Who gets chronic prostititis?
Chronic prostititis is a very common disease among men.
9 in 10 men with chronic prostititis have chronic prostititis or CPPS.
1 in 10 men with chronic prostititis have chronic bacterial prostititis, so this class of prostititis is rare compared to chronic prostititis or CPPS.
What causes chronic bacterial prostititis?
Chronic bacterial prostititis is an infection of the prostate, it is caused by a chronic bacterial infection of the prostate, the prostate symptoms are recurrence urine infections. The prostate gland is a perfect hiding place for infection and therefore recurrence of infection occur. Chronic bacterial prostititis is regarded by mainstream prostate dr. as not a sexually transmitted infection. However, our prostate dr. don't agree with that and we think the infection is major cause of all prostititis.
What are the symptoms of chronic bacterial prostititis?
Symptoms of chronic bacterial prostititis usually linger for a long time, during a flare-up, pain and symptoms of the prostate start to affect you at the base of penis, around anus, public bone, lower back, sometimes passing stools can be painful and difficult, urinary symptoms and infection such as pain during urination, frequent and urgent urination to pass urine.
Pain and symptoms are similar to the symptoms of acute bacterial prostititis, however, men with recurrence of chronic bacterial prostititis tend to less ill than those with acute prostititis, for instance, a fever is highly unlikely and you are also less likely to have aches and pains.
For chronic bacterial prostititis, your prostate symptoms will generally get relieved when treated with antibiotics, however, this is not always the case, unless the antibiotics completely clear the infection in the prostate gland, you are at risk of the infection coming back again in the prostate. Sometimes, you may still have some prostate pain and urinary symptoms.
What causes chronic prostititis/chronic pelvic pain syndrome?
Chronic prostititis or CPPS is a chronic pain in the prostate and lower pelvic area, it is usually diagnosed and classified as chronic class if you have had pain for three months or more.
Nowadays, the real cause of chronic prostititis or chronic pelvic pain is still under debate and not fully understood, many theories have been suggested as to the cause, these include: infection of the prostate with bacteria not yet been identified, nerve system problems affecting the prostate, an autoimmune problem of the prostate gland which means antibodies that are produced to fight infection may be attacking the cells of the prostate gland for some reason, infection resulting from urine being forced backwards up into the prostate gland at the time of urination.
Prostititis really refers to the infection of the prostate gland, however, due to the stealthy nature of bacteria, the exact cause of this form of chronic prostititis is still not full known and the response to antibiotics and painkillers are not satisfactory. For this reason, some prostate dr. prefer to use the term chronic pelvic pain. This is a broader term that doesn't imply that the problem are caused from a problem with the prostate gland.
Symptoms and pain of the prostate come in different form and vary widely, sometimes these prostate symptoms and pain on and off during a long period of time.
Prostititis symptoms include:
Prostititis Urinary symptoms, such as mild pain when you pass urine, urgent urination, incontinent urination, weak urine stream.
Prostititis Pain symptoms, pain at the penis, in perineum, in lower abdomen, in lower back, pain in thigh, pain in foot. The pain may vary daily.
Prostititis Sexual problems, you may experience impotence, difficult getting erection, ejaculation is painful, and pain after sex.
Prostititis Other symptoms, general aches and pains, and feeling tired all the time.
DRE, digital rectal examination are used for examination of your prostate gland, your prostate doctor may examine your prostate with a gloved finger in rectum, your prostate gland may be hardened or tender.
Most prostate dr. may want to have a urine test on you, a urine sample is taken to check for infection, this is especially important for chronic bacterial prostititis. However, urine test is considered irrelevant to diagnosis of prostititis by our prostate dr. and our prostatitis center.
Chronic prostititis can produce similar prostate symptoms to a sexually transmitted infection, for this reason, when you suspect a sexually transmitted infection, your prostate doctor may suggest that you have some swab or urine tests to make sure you don't have STDs.
Other tests for prostititis may be used to rule out other medical conditions of the prostate or other genital organs if the prostate symptoms are not typical, your prostate dr. may suggest blood test to check for other problems with the prostate gland.
If prostate dr. suspects that you have chronic prostititis, they may refer you to a urologist for further prostititis treatment. During urologist visit, prostate fluid is collected from the prostate for prostate tests. To collect prostate fluid, prostate dr. vigorously massage your prostate with a gloved finger in your rectum, prostate fluid is pushed out into the urethra and comes out from the penis to be collected and tested for bacteria. If you have CPPS, bacteria is also found in the prostate fluid but not in urine.
Treatments for chronic prostititis can be difficult, prostate symptoms may improve slightly over a long period of time.
During the first visit to your family doctor for chronic prostititis, they usually refer you to a specialist for further assessment, in the meantime, your family doctor may suggest some treatments while you are waiting for appointment with a urologist.
Painkiller, paracetamol or ibuprofen help ease the prostate pain.
Laxatives is helpful if painful or difficult to pass stools.
Oral antibiotics, usually a course of four weeks is helpful.
Psychological problems of chronic prostititis are also vital to the cure of prostititis. Reassurance and explanation are helpful, many patients worry that they may have serious disease such as prostate cancer, worry and anxiety can make symptoms worse, therefore, it is useful to know that you have chronic prostititis and not some other serious diseases, however, you will have to accept that prostate pain or symptoms are likely to persist for a long time. Psychological problems are largely ignored by prostate dr. when consulting with prostititis cases.
A lot of treatments have been tried for chronic prostititis, they may benefit some people, but so far no research studies could confirm these treatments help in most cases, they are not standard or routine treatments and your urologists may suggest you try one of the following:
A prolonged course of oral antibiotics, when prostate dr. thinks that you have chronic bacterial prostititis and prostate symptoms are not cleared completely after four-week course of antibiotics, they may suggest an extended course of treatment, sometimes the duration of treatment could be up to three months.
Prostate surgery to remove the prostate (prostatectomy) is another treatment option when prostate dr. runs our of options, it is not clear how much this may help but these small stones of calculus may be the cause of recurrent infection in chronic bacterial prostititis.
Oral antibiotics treatment for prostititis is used during early treatment although the effectiveness is poor, it could be that some antibiotics have anti-inflammatory properties as well or that they may clear some bacteria that are difficult to find when your urine is tested.
Alpha-blockers for prostititis may be used to treat enlarged prostate by relaxing the muscle tissue of the prostate, there is some evidence that alpha-blockers help in chronic prostititis/CPPS.
Alternative medicine for prostititis such as bioflavinoids such as quercetin, and finasteride that could shrink the prostate.
Prostate Psychological consultation for prostititis stress management and other pain relieving tools are also considered to help with the persistent prostate pain.
There are many research ongoing in different countries to try to find real treatment for chronic bacterial prostititis and chronic prostititis/CPPS.
Diagnosis on prostititis is difficult, treatment on prostititis is difficult, prognosis on prostititis is grim, prostate pain and symptoms may last for a long time although they may come and go or vary in severity.
Most chronic prostititis or CPPS cases tend to have some improvement in prostate pain and symptoms over the following six months, in one prostititis study, about a third of men had no more symptoms after one year, one-third of men have moderate improvement after a few years.
